Case Management - Rule 79 – This voluntary service offers assistance to seriously and persistently mentally ill adults in obtaining necessary mental health social services, 金融援助, 医疗, 法律, 职业, 住房, and other services designed to facilitate their adaptive 社区 adjustment. Children’s 澳门十大赌博正规官网app Case Management is a service provided to assist families in: determining if any mental health problems exist, determining what types of treatment or services can help deal with these issues, arranging necessary services, making follow-up contact to ensure that needed treatment is being provided.
Case management services are voluntary and include assisting the family in obtaining a comprehensive diagnostic assessment, 如果需要, developing a functional assessment, developing an individual family 社区 support plan, and assisting the child and the child’s family in obtaining needed services by coordination with other agencies and assuring continuity of care. These collaborative suicide prevention strategies help to fulfill the following goals:
- Goal 1: Increase individuals, 组织, and the 社区’s capacity to develop and implement a comprehensive public health approach to suicide prevention.
- Goal 2: Promote factors that offer protection for suicidal experiences across the individual, 的关系, 社区, 以及社会层面.
- Goal 3: Identify and support individuals who are experiencing mental health challenges or who are having suicidal experiences.
- Goal 4: Strengthen access and delivery of care for mental health and suicide.
- Goal 5: Connect, heal, and restore hope to those impacted by suicide.
DVHHS has explored several strategies to help reduce the risk of death by suicide within Jackson and Cottonwood Counties by choosing to focus on raising awareness of existing resources 资源 and 推荐指南 and providing additional support to vulnerable populations. Strategies include sharing information with cross-sector partners such as law enforcement and health care providers on best practices and training opportunities, collaborating with 社区 partners such as mental health providers, and coordinating educational materials for the school districts (parents, 学生, 员工), 雇主, and 社区 members.
